What this policy covers. This explains how we collect, use, and share consumer health data you provide through this page, as required by the Washington My Health My Data Act. It is separate from any general website privacy policy.

What we collect. When you fill out our form, we may collect: your name and contact information; the name and details of the person who may need care; and health-related information you choose to share about that person’s care needs (for example, medical conditions, mobility, memory care needs, medications, or level of assistance required). This health-related information is “consumer health data” under Washington law.

Why we collect it. We use this information to understand the care needs described, to help you connect with Normandy Park Adult Family Home, and — if you ask us to — to help find another appropriate care home.

Who we share it with. With your consent, we share the information you provide with Normandy Park Adult Family Home so they can follow up with you about care and placement. If Normandy Park Adult Family Home is not the right fit, we will ask whether you would like us to help you explore other care homes. We will only share your information with other homes if you ask us to. We do not sell your consumer health data, and we do not share it with advertisers or data brokers.
